This is a secondary phenomenon from straining or what is commonly called a valsalva maneuver.
My diagnosis is that as you push to have a bm, the nerve is being pushed into either a disc herniation or spur in your lumbar spine. Mri of the lumbar spine is indicated and f/u with your doc.
