In brief: Always
A patient should be consented before any invasive procedure.
Included in this process is a discussion of risks, benefits, and alternatives to treatment and refusal of treatment.

In brief: Yes...
Normally, the treating physician is concerned about the ultrasound report showing "suspicious" findings.
I would think he/she would discuss the findings with patients before suggesting FNA.

In brief: Yes
All test results should be discussed with a patient to inform and consent the patient before the next step.
If it is suspicious one needs to know that fna (needle aspiration) may not be positive but does not exclude cancer for sure and excision may be needed anyway.
