In brief: No single choice

Choice of antibiotic first requires proving infection and antibiotic sensitivity the choice then depends on specific bacteria or all the different ones present as well as proven sensitivity on culture.

In brief: Depends

This depends on the location and bacteria causing infection.
Most skin / wound infections are gram positive bacteria - so antibiotics to target this is used. If a patient has special circumstances (like immune system issues or certain medical conditions)- he/she is susceptible to uncommon bacteria. Antiobiotic should be tailored to the site and suspected bacteria or cultured bacteria.
