How do I bring up the possibility of depression or bipolar disorder or ADHD to a doc? I'm recovering addict and am trying to better myself.

Just ask. Addict or not the only way you will know is to discuss all of your symptoms with your doctor. Only difference is (s)he is unlikely to prescribe any medication that may be in any way addictive. Many medications can be used which may help you in the recovery process, especially if your addiction is due to self treatment of underlying symptoms.
