This is not Parathyroid adenoma with elevated ACE level it is likely Sarcoidosis, which is controlled often with steroids. An elevated PTHr-P may also be seen in sarcoidosis.

Hyperparathyroidism can be secondary to chronic kidney disease or primary with adenoma or hyperplasia of parathyroid gland.
Ct scan and pet scan may not show abnormality. In nuclear medicine we perform early and late imaging with tc99m sestamibi, parathyroid scan. If localize adenoma or hyperplasia sometimes surgically removed with relief for patient. Should consult doctor.

PTH r-p is elevated not parathyroid hormone. this is not hyperparathyroidism.
