What would cause severe pain in three stomach that radiates to the back. My wife has this problem. She can hardly function when it hits?

In brief: Pancreas/gallbladder
There are multiple causes but i would see an doctor and ask them to check her gall bladder for gallstones.
When gallstones pass, they can also irritate the pancreas so this should be checked as well. The stomach or esophagus can also give pain that refers to the back and they can be evaluated with an endoscopy if necessary.
