On recent blood tests i had a bun of 46, creatinine of 1.6 and GFR of 37. Have had diabetes for 53 years. How concerned should I be?

You are quite fortunate that for a woman with 53 years of diabetes, who is a type 1 who developed it at 10 years of age, now 63.
You have stage 3 chronic kidney disease (ckd). You had excellent glucose and blood pressure control to have such a good level of ckd. If you not have one, get a nephrologist to follow up as you may need some treatment soon for anemia of ckd and hyperparathyroidism.
