In brief: Likely concussion

The tinnitus and dizziness could be from a concussion especially if it is not localized to one ear. Concussion symptoms will improve over time.
If it is one ear then you might have labyrinthine concussion where the force delivered to the temporal bone that houses the ear causes hearing loss, tinnitus, and vertigo. See ENT if just one side. More importantly is why u passed out. Discuss with pcp.
