In brief: Relatively safe

Hydrocodone is widely prescribed in nursing mothers as an effective pain med with minimal effect on the baby.
Doses of up to 5mg every 4hrs seems to have minimal effect on baby.It is reasonable to watch baby for sedation or constipation. See medications & mother's milk, t hale, 14ed.2010.

In brief: Limit the amount

After 2-3 day as milk comes in the amount of medicine passed through the breastmilk increases and so does the concerns for increased sleepiness, lethargy, breathing difficulties, feeding difficulties, etc.
It is best to limit the amount to the lowest dose necessary, not to exceed 30 mg daily and supplement with non-narcotics.If your baby exhibits any of the above symptoms, contact your pediatrician.
