In brief: PT?
It isn't that common to get blood clot or hematoma under the skin and for it to start hurting 3 months after surgery.
Make sure the ultrasound was pelvic and looked at ovaries/uterus as well as abdominal and looked at liver, subcutaneous tissues, etc. If no obvious culprit, physical therapy and/or acupuncture can be very helpful.

In brief: Hematoma?
If ultrasound shows a collection of blood ("hematoma", or "blood clot") in the incision site, this may resolve on its own.
It depends on the size and location. Sometimes it is helpful to evacuate a post surgical hematoma, but that involves another surgery. Her obstetrician should have a lot of experience with this and will be able to advise.
