In brief: Difficult issue...

Metrnonidazole by mouth is almost always effective in improving symptoms in the short run, but recurrence is common.
Avoiding sex or consistent condom use is helpful. Female (lesbian) partners must be treated. Sometimes brief treatment (e.g. 2-5 days) once a month with metronidazole by mouth or vaginally (MetroGel) is helpful. Never douche, which can cause BV and makes it harder to treat.
