In brief: Sore Throat Continue
A sore throat for weeks needs to be evaluated by your doctor or best an ENT doctor. I suspect a chronic irritation, such as acid reflux or mouth breathing as the two most likely causes, but always want to obtain a complete exam of the upper airway to exclude something more dangerous.
Go see an ent.

In brief: Sinuses/acid reflux
This could be 1)allergies and sinus drainage 2) a sinus infection with sinus drainage 3) acid reflux or 4) chronic tonsillitis.
If there is a history of smoking and/or drinking with chronic sore throat.. It should be checked by your local ENT to rule out cancer.
