In brief: Many functions
Metformin continues to impress us as physicians with its ability to restore many endocrine functions gone awry.
Some of its mechanisms we still dont know but for the most part and particularly in women with pcos and Insulin resistance component, metformin helps in decreasing Insulin resistance which in turn will lower total Insulin production and reduce ovarian production of testosterone.

In brief: Insulin sensitivity
Resistance to Insulin (ir) causes the pancreas to make lots of insulin.
This increase interferes in the normal menstrual cycle and also warps lipids. Additionally, the overworked pancreas is at risk for wearing out, leaving one with a high risk for developing diabetes. Metformin decreases ir which helps the body need less insulin.

In brief: PCOS and sugar
A majority of women with pcos have normal sugar levels and normal fasting insulin.
It is usually only after a sugar load is the marked Insulin resistance evident. Too often physicians suggest a women won't benefit from metformin because glucose is normal. That is not true. Best results happen when metformin is combined with exercise and low glycemic diet. www.ivf.com/pcostreat.html.

In brief: Glucose/insulin
Part of the problems caused by pcos can be elevated levels of glucose (sugar) and Insulin (the hormone that regulates glucose), both generally in the body and locally in the ovary.
Metformin is a diabetes medication that helps decrease Insulin and glucose levels, thereby improving your ability to ovulate (and possibly the quality of the eggs too).
