In brief: Biopsy is best

The gold standard (best test) is usually a biopsy from your intestinal track.
But that's not easy to do. Nowadays there are several blood tests that can help us diagnose it. But as with any blood tests they are subject to error. It can miss certain cases or even be falsely positive.

In brief: No

You can certainly have it & never test for it.
Same for syphilis, heart disease or cancer. It is never advisable to assume you do, follow a gluten free diet & never seek proper diagnosis. This condition is often complicated by other medical conditions that can be found early if you know for sure. The diet is also somewhat burdensome if done right & prone to cheating when you are not sure it is needed

In brief: Yes

The diagnosis of celiac disease is usually confirmed by detecting antibodies in a blood test done while eating foods containing wheat.
