A disc herniation would be one option. A muscle pull in the calf another. I suggest rest, ice, elevation. If not better in 5 days see your md.

More information needed: when did it start, is it just one side? Is it associated with swelling or redness, any history of DVT (deep vein thrombosis), any history of pulmonary embolism (ipe - blood clot in lung) any history of cancer? Any trauma or muscle strain?
