When I go to the gym, walk outside or ride my bike I get severe pain in my right ear and right temple/jaw. What could this be?

It's hard to say based on what you are saying, but each of those activities involves exertion.
.. And possibly clenching of teeth (unkowningly).. Try some Motrin (800mg) three times a day for about a week. If the pain resovles.. Likely a TMJ issue - see your local ENT or dentist for further evaluation.
