There may be bacterial infection coexisting with a skin problem such as contact dermatitis (allergic reaction), dyshidrotic eczema/hand eczema, fungal infection, or pustular psoriasis.
You should see a dermatologist for evaluation, possible cultures and/or biopsy, and treatment recommendations.
This represents probable infection or allergy which needs medical treatment.
Over the counter medicines will not be effective for a case like this. It includes several possibilities- dyshidrotic eczema, contact dermatitis, pustules psoriasis.
