One possibility is that the boy has GERD with reflux causing asthma, dysphagia and abdominal pain. Trial of antacids and raising the head of the bed six inches and trying to keep head well above the stomach. Also food sensitivity, milk allergy and lactose intolerance should be considered. Try completely off all COW'S MILK PRODUCTS including milk, cheese, yougurt etc. See GI and allergy docs.

common things are still common even in a child with as many problems as yours - constipation and gas can cause both distention and pain.

In brief: Angioedema

Severe abdominal pain with distension suggests maldigestion generating gas (usually co2).
Accompanying symptoms include flatus & diarrhea. Most common cause is lactose intolerance. Avoidance of all dairy for 1 week should give you the answer. A rare but overlooked cause is angioedema due to c1 esterase deficiency. 50% are inherited, the rest new mutation. There is a diagnostic blood test.
