My male friends X-ray report: very early oa change, fullness of suprapatellar space consisten with small joint effusion. What does this mean?

Early oa change, suggests narrowing of the joint space, and signs of early arthritis.
Effusion is the medical term for fluid. Not unusual to have some fluid anytime there is inflammation. But remember, inflammation can be from many different things. Arthritis, gout , infection (many more ). Up to the doctor, to evaluate the knee and determine why there is fluid. Best wishes.

The wear-&-tear variety (osteoarthritis); patella is kneecap; small collection of fluid (effusion) in the space above the patella.
If friend is overweight, encourage weight loss to ease stress on ankles/knees/hips. Tylenol (acetaminophen) or nsaids (aleve/advil/motrin/etc) can relieve discomfort. If effusion gets too large, may need to be drained.
