In brief: Angular cheilitis
The cause is secondary to overgrowth of microorganisms at the corners of the mouth (usually the yeast candida albicans).
The treatment is to treat the yeast with medicine such as Nystatin and/or by addressing some of the causes which lead to the overgrowth in the first place (excessive saliva pooling at the corners of the mouth, lip licking ). See your dermatologist and/or dentist for evaluation.

Yeast (a type of fungus) flourishes in a moist, warm and dark environment such as the corners of your lips (oral commisures). Alternatively, the fissures can be a result of bacterial colonization. If an antifungal cream doesn't cure the problem, then try an antibiotic ointment or cream. Avoid licking your lips!

Definitely worth trying treatment with topical medicines (antifungal or antibacterial ointments) after you have it looked at by your physician.
