Depending on the doc, a vaginal breech extraction can be performed on twin 2 if twin 1 paves the way.
If vtx/ breech it would likely b #2 a breech extraction. Most docs go for the section because we have less experience with breech delivery. At times it's no so clear, baby 2 may be transverse and come down either way. Most docs go for the section on twins unless both r head down, not unwise.

When you start contracting and any of the twins is still in malposition (means, not a head down), the decision would be: c-section.

What ever position the second twin is in, it can flip and come down in some other position.
