If doing all of the above fails, then antibiotics are warranted to prevent the infection from getting worse and spreading to the kidneys or blood stream. Untreated urinary tract infections can become very serious especially if the person has other medical conditions such as diabetes.

Antibiots after getting a urine culture. Drinking water and cranberry juice should be ancillaries.

Agree with above; if you have active UTI need antibiotic treatment. If you are trying to prevent (prophylaxis) additional UTIs given a history of having recurrent UTI, but do not currently have a UTI, then cranberry pill extract (OTC) is worth a try or a low dose antibiotic (i.e. macrodantin) can be prescribed by your urologist after discussing the risks and benefits.
