In brief: Anxiety
Feeling of anxiety are very distressing and have several causes.
You need to see your doctors to have therapy for the real cause of your anxiety and continue therapy for how ever long it takes to get better.

It may be that your disorder has not been clearly defined or the treatment needs changed, maybe both are true. Two years is long time to feel terrible. There are many causes of anxiety and a variety of treatments as well. Surely in that mix there is a way through this. Don't be afraid to ask your therapist, counselor or doctor the same questions you are presenting here. They can help.

In brief: Long term disorder
Anxiety disorders are often chronic.
However, if you are not seeing improvement it may be that you are not receiving the proper treatment. Many symptoms of anxiety respond well to behavior therapy and to cognitive reframing. At times an individual is unable to do this type of therapy without the aid of psychotropic medications. These must be prescribed by a physician.
