In brief: Plaque rupture

Most are progressive narrowing to the point of serious impairment of coronary blood flow with exercize etc.
Causing angina or heart attack some are acute thrombus on a plaque or elevation of the plaque like a trap, door- dissection or rupture of the built up area of disease.

In brief: Hard to say

Ischemia means a lack of oxygen to the tissues.
This is usually caused by a blockage in the supplying artery. Blockages are most often caused by atherosclerosis which is a slowly progressive disease. Sometimes the plaque (diseased part) can break off and suddenly occlude the vessel. Also the disease can cause symptoms which are ignored until it is too late.
