You could also consider a pain management specialist. Choose one who can offer you injections rather than just medications.

If after all you are doing you are still in pain, you should consider seeing a fellowship trained spine specialist who would need to evaluate if your care was done correctly and who may be open to the idea of other options such as prolotherapy, stem cell therapies and maybe even targeted/specialised physical therapy.
Many times these newer treatments are not covered under insurance so be prepared

After thorough evaluation and testing, simple measures like anti-inflammatory drugs and newer drugs like duloxetine may be effective. These may be followed by epidural steroid injection or facet radiofrequency denervation of nerves may be needed

Orthopedic specialists have no special juju with back pain. Even ortho spine surgeons have problems with patient selection. 5 years out, no difference between no surgery and fusion with regard to back pain. your FP, if properly trained can refer U to physical therapy just as accurately as ortho 4 half the price. OMT, Postural & core strengthening exercises, stop smoking adjusted motion .......
