Most of the testing on new chemotherapy drugs centers on 2 things: the effectiveness of the drug against the particular cancer and the severity of side-effects that is causes.
We approach treatment of all illnesses in a risk/benefit approach. Does the benefit of using a particular treatment or medication outweigh the risks that it may cause. Most doctors are conservative and follow this approach.

The chance of a second primary malignancy (SPM) varies by age (younger = more time to develop SPM), primary cancer (some more tumorigenic), genetic risk factors (eg, BRCA1/2 mutations, DNA repair mutations), and treatment type. A recent 12/13/11 NEJM article suggested that we can reduce radiation exposure in some HL pts to reduce SPM. So, over time we may be able to improve risk/benefit ratio.
