In brief: Halitosis

Bad breath (halitosis) can have multiple causes.
In addition to brushing, it is important to floss. Gastro-esophageal reflux (can cause symptom of heartburn) can also contribute to bad breath as can chronic sinusitis (not enough space to discuss treatment for these; please search healthtap for these issues). Liver/lung/kidney disease also can contribute to bad breath.

In brief: Mouth breathing

Sleeping with your mouth open is another common cause of bad breath.
Do you snore or have chronic nasal congestion? These symptoms may be the result of nasal obstruction from allergy, nasal polyps, deviated nasal septum, or enlarged adenoids. You may wish to be evaluated by a board-certified otolaryngologist or allergist or by your primary care physician if you have persistent bad breath.

In brief: Many things!

Brushing is only one part of oral hygiene.
Flossing is crucial, you miss 30% of your tooth by not flossing! i would also highly recommend a tongue scraper and daily use of an antibacterial mouth rinse such as listerine or crest pro health. See you dentist to ensure you do not have periodontal disease. As mentioned above eliminate other contributing factors such as gerd/acid reflux.
