Doctors can you tell me when should a diabetic go to the hospital, when their glucose level is over 300, 400, 500, or higher?

In brief: When he is lethargic
It depends how dehydrated the patient is.
If your blood glucoses are high and even by giving extra Insulin or extra oral meds the bloos glucose remain high or the patient start getting ketones in the urine the he or she should go right away. It doez not depend on the level of the blood glucose only it also depends on overall patient condition and level of hydration.

In brief: Reading and others
The criteria for hospitalization or medical evaluation in diabetics will depend on several factors beside glucose reading such as high urine/serum ketones, intractable vomiting or persistently elevated bs despite aggressive Insulin administration.
Try to establish care with a diabetes specialist in order to have a backup plan as most diabetic emergencies can be prevented.
