When widening of blood vessels is noted on plain x-ray studies of the back or abdomen we mostly are referring to the abdominal aorta or its' major branches such as the renal, bowel, or iliac arteries.
This could suggest an aneurysm and if so then further evaluation would usually require ct or ultrasound.
