In brief: Find cause

If the anemia is significant, it is likely that at least some of the foot swelling is due to high-output heart failure.
There's often little to do for a diseased heart, but it is almost always possible to treat an anemia if the cause can be found. The thrombocytopenia may also be a clue. Physicians know how to do these workups. Good luck.
