ACL reconstructed with HS autograft 6 months ago. Last week it swelled up, I didnt feel any pain. Is it ACL failure or a meniscus tear?

Hard to say. Swelling in absence of pain or traumatic event could be any number of things but definitely less concerning than these same findings in assoc w/ pain or twisting re-injury to knee.
