I had a test result of trace of kidney disease I am a dibetic. What measures shall i take to prevent kidney disease. Is it reversible?

In brief: Glucose and BP

Presumably you have microalbumuria (elevated urine protein).
Diabetic kidney disease can be mostly stopped by controlling glucose (a1c as close to normal as possible), controlling bp, an ace-i or arb (even if BP normal), controlling lipids, weight loss if you are overweight may also help, but the big things are a1c and bp. If you are a type 1 diabetic, consider a pump or intensive insulin.
