Are solumedrol (methylprednisolone) injections into the muscle painful? Should I ask for some lidocaine mixed in with it?

In brief: Yes. No.

Lidocaine itself can be painful/burning when injected.
A single injection of a steroid should not be any more painful than an injection with Lidocaine alone or Lidocaine mixed with the steroid. It is not typical to mix it with lidocaine.
