Depends on extent of arthritic damage on x-rays, and level of functional disability.
If arthritis is already severe on x-rays and in terms of function, then proceeding straight away with replacement is sometimes the smartest approach. If arthritis is mild to moderate, then of course replacement should be last option, after arthritis meds, therapy, bracing, and injectable options.
