In brief: Get it checked.

Diabetes does not cause swelling directly, but can predispose you to a number of conditions that could lead to scrotal swelling.
Infections of a testicle or epididymis can cause warm, tender swelling on one side. Kidney damage, or diabetic nephropathy can lead to scrotal edema. If you have swelling of the scrotum, or feel new asymmetry between the testes, get checked by your physician.
