In brief: Many Causes..

There are many reasons for difficulty sleeping, and several types of insomnia.
Nearly 70 million people suffer from sleep problems. If you have been having difficulty sleeping for some time (two or more weeks), it may be helpful to consult your doctor or a mental health professional to explore further. Here is a link to some tips I have developed for my patients: http://tinyurl.Com/cuexb6u.

In brief: Doctor can evaluate

Insomnia has many causes, so an evaluation by a primary care doctor is needed.
Getting rid of a problem causing insomnia will usually work, but sometimes no cause is found. Doctors may give medications to help fall asleep & stay asleep. Depression, anxiety, hormonal imbalances, pain, medicines, ambient noise, tinnitus, jet lag, etc... All can cause insomnia. "sleep medicine" doctors can also help.

In brief: Depends

Depends on if the insomnia is due to stress, psychiatric disorder, a primary sleep disorder, another medical condition, medications or poor sleep hygiene.
The best treatment is addressing the reasons for the insomnia. Chronic insomnia is best treated with a behavioral program called cbt. Medications are usually indicated for short term use.
