The incidence of lung cancer for someone 21 years of age is essentially zero.
I remember reading some where that the risk of a 30 y/o person dying from lung cancer over the next 10 years is 0. Chest xrays are an excellent screening tool. They will miss some small percentage of lesions because of size and/or location. The most important thing you can do is stop or never smoke.

Biopsy and Histology of lung tissue can be confirmatory. CXR is helpful, but not confirmatory.
