In brief: Sounds viral

Viruses are more likely to affect many systems of the body at once however this sounds like a teenager should be seen for a possible infection from a bacteria.

In brief: Influenza?

Sounds a lot like the flu.
If his neck is stiff, (ie has trouble touching his chin to his chest without pain), he needs to be seen in the er immediately. If he has a rash or has been exposed to ticks, he needs to be seen. If he has friends or family who have been sick & he has none of the above danger signs, you could try some tylenol (acetaminophen) or advil, clear liquids and wait.
