Be honest and tell her how you feel.
It is not always easy to care for older parents , especially if you have your own immediate family responsibility (i.E wife and kids), but if you tell your wife that you feels responsible to care for them because they cared for you when you were a child, but you are struggling , she probably will help you do what is right , she may even share the burden.
