There are many reasons to remove a device and leads, most common due to an infection in the heart, or other complications such as blood clots. More frequently due to the number of implants lead malfunction and recalls are becoming more frequent cause for extraction.

Usually once a device is put in we do not remove it.
The reason it was put in is usually permanent. If the device became infected it would have to be removed. If the battery wears out, it would be removed and replaced with a new one. Also if the electrode (wire) were to break or stop conducting, it might be removed and replaced with a new wire.
