Some birth defects, like spine problems can be reduced by taking your prenatal vitamins.
Others, like fetal alcohol syndrome can be completely prevented by no alcohol in pregnancy. Most though can't be predicted or prevented.

Take Folic Acid at 1-5 mg daily for at least 3-12 months before conception to decrease spinal and cardiac defects by 95 and 75% respectively.
Control your diabetes mellitus, avoid valproate/alcohol/radiation and all other teratogens, assure normal thyroid function and you can prevent the majority of birth defects.
