In brief: Removal

A rupture spleen requires urgent/emergency removal of the organ.

In brief: It depends.......

If the patient is hemodynamically stable, we will watch them for signs of bleeding.
If they remain stable, non-operative therapy is preferred. If they are unstable, we either take out the spleen (really unstable) or embolize the spleen (transiently stable) with gel-foam or coils and our radiology colleagues.
