In brief: There isn't just one

Sometimes none. Other times diuretics, ace inhibitors or beta blockers .
...Really depends on situation.

In brief: It depends

It depends on which valve is leaking.
Generally, the left sided heart valves (aortic valve and mitral valve) require directed therapy early on. The treatment is mostly directed at decreasing the pressure against which the heart works, the blood pressure. So, in general, i try to keep my patient's blood pressure low, hopefully to decrease the amount of the leak, and delaying surgery.
