In brief: Heart Damage
A "q wave" is seen on an electrocardiogram (ekg) several of which are recorded during a stress test.
Q waves may be very small and not carry much significance or may be larger and suggest that heart damage by a heart attack has occured at some point in time. Further testing and imaging of the heart may be required to determine if the q wave is significant.

In brief: Q waves
Q waves can be a sign of an old scar from previous heart attrack, or in many cases it is a normal variant.
It is not typically one of the things that we look at to determine if there are problems with blood flow to an area of the heart during exercise.
