Biggys are smoking, obesity, and add salt intake to the above - of course there are about 10 people in the country with some sort of disease that causes "primary" hypertension - ie its not their fault - now, once you have hypertension - you end up with some sort of kidney disease that sort of makes it permanent - so those people moan "but i dont eat salt".
