In brief: Different types
Cardiomyopathy (weak heart muscle) is most often caused by prior heart attacks leaving portions of the heart muscle damaged.
However, this condition can occur due to longstanding (poorly controlled) hypertension or diabetes, as well as from alcohol or Cocaine abuse or HIV infection. Other causes include viral-induced inflammation of the heart, or genetic abnormalities (i.e. "familial").

In brief: CAD risk factors
Most common form is ischemic, and therefore people with underlying risk factors for heart attacks and cad.
Otherwise, there are several patients who develop an unfortunate viral cmp, which can affect anyone and it is unclear as to why some are predisposed to this disease.
