In brief: Blood

Blood on the outside of stool or seen only on the paper suggests hemorrhoids.
Bloodmixed with stool suggests a bleeding source higher in the bowel, which can in turn represent infection, inflammation, vascular malformation, and tumor among other causes. If you are seeing blood on your toilet paper on a regular basis, or if you have blood mixed with stool, you should see a gastroenterologist. You will probably need a colonoscopy.
