but sometimes the fabric rubbing on the diaphragm of the stethoscope can cause artifactual sounds that can obscure subtle heart and lung sounds.

I try not to have more than one layer of clothing between skin and the stethoscope and skin to skin is preferable. Doctors will usually try to protect patient modesty which is often difficult when trying to get a complete exam.
