For barrett's without dysplasia, or low grade dysplasia, acid reduction and endoscopic surveillance is fine.
For small flat areas of high grade dysplasia, endoscopic options include endoscopic mucosal resection and radiofrequency ablation, for long segments of high grade dysplasia and very nodular high grade dysplasia the risk of hidden cancer is high and esophagectomy is the best option.

This is a change in the esophagus in response to longstanding reflux.
It is a precancerous change which requires frequent surveillance EGDs to make sure cancer is not developing. If Barrett's is found, the abnormal areas can be ablated so that they do not progress to cancer. Surgery only really needed if cancer is found.
