Lithium, lamictal, and atypical antipsychotics (seroquel, zyprexa, risperdal, geodon, (ziprasidone) abilify) are probably the first line agents for bipolar depression.
Antidepressants can make bipolar depression turn into bipolar mania in some people, and should probably be reserved for situations in which the first line agents have failed.

Antipsychotics and siezure medications are the other meds that treat bipolar disorder, lithium helps some with depression and is the only med proven to actually prevent suicide.Newer atypical antipsychotics help reduce depression in major depression and bipolar depression.

All the therapies for bpd have varying degrees of success, singly, or in combination. Lithium has been useful for many decades and should be tried if other treatments don't work. Usually, though, we don't initiate treatment using lithium as the 1st medication because of its toxicity.
